Job summary

Cera in Bangor is looking for a Care Coordinator to join its domiciliary care team. The role involves organising visits, supporting staff and maintaining strong client communication to ensure high-quality care.

Key duties include matching carers to clients, rostering, onboarding, and ensuring up-to-date care plans, while complying with CQC standards. Driver with vehicle preferred; NVQ/training provided and benefits include pension and DBS.

Qualifications

• Previous experience in domiciliary care coordination (preferred).
• Must be a driver with a vehicle.
• NVQ and in-house Training for your role.

Responsibilities

• Coordinate and schedule care visits using care management systems.
• Match carers to service users based on skills and preferences.
• Handle rota planning, including sickness and emergency cover.
• Communicate with clients, families and care staff professionally.
• Ensure care plans are accurate and up to date.
• Support onboarding and training of care staff.
• Monitor compliance with CQC standards and company policies.
• Handle enquiries and concerns in a timely, empathetic manner.
• Maintain accurate records and reports.
