Aufgaben

• Be the go-to patient transport expert on-site in the Discharge Lounge and Transport Office.
• Collaborate with hospital teams to streamline patient flow and prioritise transport needs.
• Carry out risk assessments as required.
• Represent transport interests at daily bed meetings, aligning plans with critical discharge timelines.
• Maintain clear communication between wards, patient flow teams, and transport providers.
• Keep patient records accurate and secure, ensuring smooth transitions and compliance with data protection policies.
• Provide support to patients, families, and care facilities, ensuring everyone is informed and prepared for discharge journeys.
