Job summary

First Care Ambulance in Devon is seeking a Patient Discharge Co-ordinator to join our team at the Royal Devon Hospital. This full-time role focuses on coordinating timely patient discharges and arranging transport for post-treatment care.

You will work with patients, medical staff and HTG colleagues to manage discharge pathways, resolve issues, and maintain high-quality service. The position offers two weeks of training, a full uniform, and ongoing support.

Qualifications

• Strong organisational skills and ability to manage multiple priorities under pressure.
• Excellent communication and relationship-building skills with stakeholders at all levels.
• High attention to detail and commitment to confidentiality.
• To hold a Full UK Manual Driving Licence, with a willingness to travel if required.
• Problem-solving skills to handle real-time issues and create practical solutions.

Responsibilities

• Coordinate and manage patient discharges through effective communication with healthcare teams and First Care staff.
• Liaise with hospital discharge points to maintain patient flow throughout the day.
• Manage expectations within the discharge facility and assist with complex discharges.
• Respond to patient queries and provide information on First Care services.
• Conduct risk assessments and ensure compliance with company policies.
• Work collaboratively with multiple depots to meet logistical and contractual requirements.
