Job summary

Slough Borough Council is hiring 2 Occupational Therapists for the Discharge to Assess (D2A) team. The posts focus on enabling independence through strength-based assessments and interventions to ensure timely discharges from hospital to home.

You will work with hospital social work, reablement services, and MDTs in a fast-paced setting and may involve community/home visits and occasional hospital or care home placements.

Qualifications

• Enhanced DBS clearance is required.

• Join the Discharge to Assess (D2A) team and support safe, timely hospital-to-home discharges.
• Collaborate with MDT of health and social care professionals to streamline discharge pathways.
• Contribute to strength-based assessments and care planning, including in-reach work to care placements.

Job description

• Directorate: Adult


• Pay Level: 7, £43,253 - £48,795 (fully inclusive of Local Weighting Allowance) plus (14% Shift enhancement)


• Contract Type: Permanent


• Working Hours: Rota working across 7 days a week including weekends, bank holidays and out of hours as required. Rotas are subject to change based on the needs of the service.


• Location: Observatory House/ Slough Hybrid (minimum 2 days per week in the office). The role includes working from people's homes in the community and visiting hospitals or care home setting if required as part of the discharge planning process.

What you'll be doing:

We are hiring 2 professionals to join our dynamic Discharge to Assess (D2A) team, where you'll work closely with the Hospital Social Work Team and the Reablement and Independence Service to support individuals transitioning from hospital to home. This role focuses on enabling service users to regain or maintain independence through strength-based assessments and interventions, ensuring safe and timely discharges.

You'll collaborate with multi-disciplinary teams (MDT) of health and social care professionals in a fast-paced, supportive environment that values your clinical expertise, innovative ideas, and continuous professional development.

As a D2A Occupational Therapist, you'll contribute to streamlining discharge pathways, reducing delays, and promoting single-handed, proportionate care planning. You'll also support residents in care placements through in-reach work, ensuring continuity and quality of care.
