Qualifications

• Level 3 Diploma in Adult Care (RQF) or equivalent.
• Level 4 Diploma in Adult Care (RQF) or equivalent.
• Experience of working in a team and within an integrated team.

Responsibilities

• Assess individuals' ability to engage reablement and support discharge to assess pathways.
• Liaise with hospital staff, service users, carers and other professionals to coordinate care.
• Represent the organisation professionally and provide advice with health & social care judgement.
• Work independently using assessment and evaluation skills to adjust care plans.
