Hertfordshire Partnership University NHS Foundation Trust is seeking a Discharge and Community Liaison Social Worker to support older adults with functional mental health needs. You will lead on social care assessment, care planning and discharge coordination from hospital to community settings, using strengths-based practice and promoting independence.

Formación

• Must have professional social work qualification and registration with Social Work England.
• Experience leading discharge planning and coordination.
• Knowledge of safeguarding, Care Act 2014, Mental Capacity Act 2005, and relevant mental health legislation.
• Ability to provide expert social work assessment and advocacy within MDT settings.

Responsabilidades

• Lead social care assessment, care planning and discharge coordination for older adults.
• Coordinate discharge pathways with inpatient and community services to reduce delays.
• Provide professional advice on social care legislation and best practices.
• Maintain accurate records, documentation and supervision for staff development.
