Hertfordshire Partnership University NHS Foundation Trust is recruiting for a role within the Enhanced Discharge Team to support service users with identified needs and to facilitate discharge from inpatient wards back to the community.

You will work as part of an MDT, collaborating with families, housing authorities and charitable organisations to overcome discharge barriers, using the Support Time and Recovery Model and a patient-centred approach.

Qualifications

• NVQ Level 2 or equivalent qualification.
• Commitment to Continuing Personal Development.
• Ability to use structured therapeutic skills (e.g. solution‑focused approaches).
• Ability to acknowledge diversity and promote anti‑discriminatory practice.

Responsabilités

• Work as part of MDT within Enhanced Discharge Team to support discharge from inpatient wards.
• Collaborate with housing authorities, charitable organisations and governmental workers using the Support Time and Recovery Model.
• Support service users with identified needs to facilitate discharge back to the community.
• Coordinate with inpatient staff, bed management and care coordinators to resolve discharge barriers.
• Engage with families, carers and housing workers to meet patient needs in a timely manner.

Description du poste

The successful candidate will work collaboratively as apart of an MDT to support service users with identified needs to facilitate discharge. The candidate will work closely with service users, families, housing authorities and other charity and non governmental organizations to achieve their goals.,

• To work as part of an MDT within Enhanced Discharge Team to support with service suers with identified needs to facilitate discharge from inpatient wards back to the communities or signposting to other teams like CSPS and rehab services.
• To work in partnership with the Housing and accommodation workers, charitable organizations, governmental workers and Department of Works and Pensions providing support using the Support Time and Recovery Model.
• To work across the Adult Acute Inpatient wards in Hertfordshire as well as Contracted Independent Sector Providers.
• To work closely with the Inpatient staff, medical team, CATT team, bed management team, the housing worker, Health and Social Care Teams, Bed Placement, Care Coordinators and carers to enable barriers to discharge to be identified and resolved in a timeframe that meets the patient's needs., To work in partnership with the Housing and accommodation workers, charitable organizations, governmental workers and Department of Works and Pensions providing support using the Support Time and Recovery Model.
• To work across the Adult Acute Inpatient wards in Hertfordshire as well as Contracted Independent Sector Providers.
• To work closely with the Inpatient staff, medical team, CATT team, bed management team, the housing worker, Health and Social Care Teams, Bed Placement, Care Coordinators and carers to enable barriers to discharge to be identified and resolved in a timeframe that meets the patient's needs.
• The Trust is looking for a vibrant, motivated candidate with very good communication and engaging skills to support service users with social care issues prior to discharge from Inpatients unit., NVQ level 3 qualification or mental health certificate level 3 or agreement to undertake such training as soon as resources permit

• Good awareness of local services, Experience of using electronic recording systems and ability to produce accurate, appropriate and timely reports
