Job summary

Midlands Partnership NHS Foundation Trust in Stoke-on-Trent is offering a Discharge Facilitator role within the Discharge to Access (D2A) Complex Assessment team. The post covers discharge planning from admission through to bedded care in non-acute settings, working closely with the multi-disciplinary team to ensure timely and safe discharges across Stoke-on-Trent, Cannock, Stafford and surrounding areas.

You will support patient flow, gather collateral histories, attend meetings, and maintain

Qualifications

• NVQ Level 3 in Health Care or equivalent
• Evidence of continuous development and acquisition of new skills and knowledge
• Experience in hospital or community settings with patient-facing duties
• Experience with clinical and non-clinical teams and relationship management
• Experience using computer-based information systems and MS Office

Responsibilities

• Gather and record information to enable accurate patient assessments and timely discharge
• Attend meetings, share and record information, escalate blocks or issues
• Prioritise workload to deliver objectives under Flow Team supervision
• Be proactive with good organisational and communication skills
• Understand local services and the Care Act 2014 principles relevant to D2A
