Kent and Medway NHS and Social Care Partnership Trust in Canterbury is seeking an enthusiastic Assistant Discharge Coordinator to join our Discharge team at St Martin's Hospital Canterbury. You will support both Younger and Older Adult services by coordinating early and complex discharges and working with the MDT to resolve barriers to discharge.

You will contribute to bed management and discharge planning, travel to non-KMMHT hospital sites, and engage with patients and carers to find housing

Qualificações

• Sound understanding of acute inpatient care.
• Good communication and proactive problem solving.
• Willingness to travel to non KMMHT hospital sites.

Responsabilidades

• Contribute actively to local Patient Flow team discussions and discharge planning.
• Collaborate with the MDT to identify discharge barriers early in the patient journey.
• Work compassionately with patients and carers to address discharge challenges, including housing options.
• Represent the Patient Flow team in individual patient review meetings.
• Support bed management operations by participating in a call-handling rota for incoming bed requests.
• The role will involve travelling to non KMMHT hospital sites.
