Job summary

mbi Medical Ltd is seeking a Social Worker to oversee hospital discharges under the Discharge to Assess Framework and Care Act. You will work in a multi-disciplinary team to support older people with memory problems who require social care due to mental health or dementia.

You will collaborate with NHS colleagues to respond to urgent assessment/monitoring needs, help prevent hospital admissions and facilitate coordinated community discharge alongside the intermediate care team.

Qualifications

• Overseeing hospital discharges under the Discharge to Assess Framework / Care Act.
• Provide social care support for older people with diagnosed memory problems or dementia.
• Collaborate with health and social care colleagues to assess, monitor and support urgent needs to prevent hospital admission.

Responsibilities

• Coordinate identification of needs and develop support plans within personal budget to maximise choice, control and independence.
• Ensure social care needs are met within eligibility criteria through partnership with service users, carers, professionals and providers.
• Support high standards of performance and continuous development within allocated resources.
