Job summary

Oldham Council seeks an experienced Social Worker to join the Integrated Discharge Team at Royal Oldham Hospital. You will assess, plan and review care for adults, guiding discharge via the D2A pathway with a person-centred approach.

You must be degree-qualified and SW England registered, with safeguarding, advocacy and cross-agency collaboration skills to support safe, independent living for residents of Oldham.

Qualifications

Experience of implementing, monitoring, evaluating and updating Care & Support Plans through person centred planning principles and relevant procedures.
Ability to negotiate with others both internal and external to secure appropriate services for Service Users, Carers and Families
Detailed knowledge of the relevant legislation, codes of practice, National Guidelines and Government initiatives connected with service delivery in social work
Knowledge and understanding of equalities and diversity issues in connection with delivering social work

Responsibilities

To undertake the assessment of support needs for Service Users, Carers and Families, screening referrals as needed
To be proactive with Safeguarding issues and follow policies if abuse is suspected
To identify, assess and manage risks to Service Users, Carers and Families
To respond to crisis situations, making urgent assessments and applying appropriate legal interventions
To ensure all information is accurately recorded and data quality maintained
To prepare, attend and participate in meetings and decision-making forums
To advocate on behalf of Service Users, Carers and Families within policy boundaries
