Oldham Council, Integrated Discharge Team, is seeking an experienced Social Worker to support hospital discharge through the D2A process. You will assess needs, plan care, safeguard adults, and advocate for residents, coordinating with multiple partners to deliver person-centred outcomes.

Role is based at Royal Oldham Hospital with a permanent contract and part-time hours (18.33 per week); salary bands reflect local terms and require SW England registration.

Qualificações

• Degree level qualification and SW England registration.

Responsabilidades

• Undertake the assessment of support needs for Service Users, Carers and Families.
• Be proactive with respect to safeguarding issues and follow policies if abuse is suspected.
• Identify, assess and manage risks to Service Users, Carers and Families.
• Respond to crisis situations and use appropriate legal intervention.
• Ensure records are updated in social care systems and maintain data quality.
• Prepare, attend and participate in meetings and decision-making forums; chair meetings where appropriate.
• Advocate on behalf of Service Users, Carers and Families within policies.
• Discharge plans are co-produced with residents and their support systems.
