Remedy Social Work is recruiting an Adults Senior Social Worker for the Hospital Discharge to Assess team in Manchester. You will provide direct support, carry out care coordination and safeguarding duties, and manage a caseload to secure positive outcomes for residents.

You will lead partnership planning with health and social care colleagues, ensure timely access to services, and maintain thorough records in line with statutory requirements.
