Qualifications

• Social Work England registration.

Responsibilities

• Carry out assessment, intervention and care planning for citizens discharged from acute hospital into rehabilitation beds.
• Provide formal and informal supervision to social workers and care assessors.
• Balance complex risks and lead discharge planning through the pathway.
