Job summary

Warwickshire County Council is seeking a social work professional to join the Discharge to Assess team within the hospital social care service. You will manage a caseload of adults, coordinate care planning, and contribute to timely discharges while collaborating with health colleagues and care homes.

You will participate in Continuing Health Care and Decision Support Tool meetings, provide strengths-based assessments, and travel across Warwickshire to support reviews and care planning.

Qualifications

• Professional social work qualification.
• Registration with HCPC.
• Adult social care experience.
• Good IT skills.
• Ability to make difficult decisions and work to tight deadlines.
• Fluent spoken English to give advice to the public.

Responsibilities

• Manage a caseload of adults, their families and unpaid carers.
• Attend Continuing Health Care and Decision Support Tool meetings.
• Plan discharge from Discharge to Assess beds through robust assessments and care and support planning.
• Complete reviews after discharge, including in care homes.
• Provide strengths-based assessments and support.
• Communicate with customers, communities, health partners and agencies.
• Travel across north and south Warwickshire depending on business need.
• Occasionally work with people from acute hospital settings.
• Work on own initiative in pressurised situations.
• Take part in team meetings and other meetings set by the Team Manager or Service Manager.
