Lancashire & South Cumbria NHS Foundation Trust is recruiting for a Designated Keyworker within the Keyworking Team to support children, young people and adults with ASD and/or Learning Disabilities in crisis.

The role provides a multi-agency, person-centred approach with an emphasis on outcomes. We encourage flexible working patterns and value experience of working autonomously and as part of a team.

Responsibilities

• Manage a caseload as part of the Designated Keyworker role.
• Provide multi-agency, person-centred support to children, young people and adults with ASD and/or Learning Disabilities in crisis.
• Work autonomously and as part of a team to deliver timely, outcome-focused support.
