Job summary

Connect2Dorset, Dorset Council's managed service recruitment partner, is recruiting an experienced Level 2/3 Qualified Social Worker to join the Preparation for Adulthood Team within the Birth to Settled Adulthood (B2SA) Service.

This role focuses on supporting children and young people aged 0-25 with autism, learning disabilities, SEND and complex needs, coordinating across health, education and social care to keep families connected to community supports.

Qualifications

  • Qualified Social Worker with SW England registration.
  • Minimum 2 years post-qualified experience.
  • Experience with Autism, Learning Disabilities, SEND or Complex Needs.
  • Experience managing complex caseloads in Children’s or Adult Social Care.
  • Strong assessment, care planning and risk management skills.
  • Knowledge of Children Act, Care Act, Mental Capacity Act and SEND Code of Practice.
  • Experience multi-agency working with health, education and social care partners.
  • Excellent written and verbal communication, including report writing.
  • Full UK driving licence and access to a vehicle.

Responsibilities

  • Manage a caseload of children and young people on the Dynamic Support Register (DSR).
  • Be the key point of contact for children, young people, families and carers.
  • Develop and coordinate person-centred support plans.
  • Work with NHS teams, education providers and social care professionals.
  • Attend and contribute to CETRs (Care, Education and Treatment Reviews).
  • Support hospital admission avoidance and discharge planning.
  • Provide updates to DSR meetings and multi-agency forums.
  • Maintain accurate case records, assessments and reports.
  • Identify barriers to support and promote joined-up services.

Connect2Dorset, Dorset Council's managed service recruitment partner, is recruiting an experienced Level 2/3 Qualified Social Worker to join the Preparation for Adulthood Team within the innovative Birth to Settled Adulthood (B2SA) Service.

This rewarding role focuses on supporting children and young people aged 0-25 with autism, learning disabilities, SEND, and complex needs who are at risk of mental health hospital admission. You will work across health, education, and social care services to coordinate support, improve outcomes, and help children and young people remain safely within their communities.

Key Responsibilities
  • Manage a caseload of children and young people on the Dynamic Support Register (DSR).

Essential Requirements
