Job summary

Alzheimer’s Society is recruiting a Dementia Adviser to join our Worcestershire team. This community-based role (28 hours per week) involves visiting GP practices, attending groups, home visits and building relationships with local organisations across North/Central/South/West Worcestershire.

You’ll support people from diagnosis onwards, tailoring information and guidance to individual needs, while travelling across Worcestershire to provide on-the-ground help and access to services.

Qualifications

• Understanding of dementia or willingness to learn about the experiences of people living with dementia and the people who support them.
• Experience providing information, guidance, advice or emotional support in health, social care, community or customer-focused roles.
• Strong listening and communication skills with the ability to build trust with diverse people.
• Ability to organise and manage workload, balance priorities and use support when needed.
• Confidence using IT systems to keep accurate and confidential records.

Responsibilities

• Build supportive, trusting relationships with people living with dementia and carers.
• Help people maintain independence, wellbeing and confidence in daily life.
• Provide tailored information, advice and emotional support reflecting individual backgrounds and preferences.
• Support people face-to-face, over the phone, and with other services; may include home visits.
• Take part in referrals and respond promptly to new referrals to ensure timely support.
• Maintain accurate, confidential records and manage your workload.
• Help people access local services, activities and networks to reduce isolation.
• Collaborate with colleagues and contribute to service development and improvements.
• Facilitate peer support groups and host community events to share information and resources.
