Job summary

Pennine Care NHS Foundation Trust in Stockport is seeking a Band 6 Mental Health Practitioner to deliver assessment, signposting and brief therapeutic interventions for children and young people in crisis. You will work across CAMHS crisis pathways including Rapid Response, Home Treatment and Enhanced Rapid Response Paediatric Liaison, with base at Heathfield House, Stockport.

Qualifications

• Core professional qualification in mental health (nursing, social work or occupational therapy).
• Current professional registration and ongoing professional development.
• Educated to first degree level or equivalent skills and abilities.
• Literacy, numeracy, IT and keyboard skills.
• Short courses and experience to postgraduate diploma level.

Responsibilities

• Provide assessment, support, signposting and brief therapeutic intervention for CYP in crisis.
• Work within CAMHS crisis pathways (Rapid Response, Home Treatment, Enhanced RAP) to support young people.
• Contribute to discharge planning and community-based care to prevent readmission.
