Job summary

Pennine Care NHS Foundation Trust is seeking a Community Mental Health Practitioner (Band 6) to join the Tameside West Specialist Mental Health Team. You will assess and manage a defined caseload, delivering evidence-based mental health interventions and risk management within a multi-disciplinary setting.

The role involves extensive liaison with partners, care planning, and maintaining high standards of professional practice across community services.

Qualifications

• Substantial post-registration experience in mental health
• Experience managing a caseload in a multi-disciplinary team
• Knowledge of safeguarding and care planning processes
• Ability to work across community settings and travel as required

Responsibilities

• Assess health and social care needs of service users
• Develop and implement care and risk management plans
• Provide evidence-based therapeutic interventions
• Maintain accurate electronic health records
• Liaise with GP, social services and other agencies
• Participate in MDT duties and supervision
• Support junior staff and care coordination activities
