Job summary

Midlands Partnership University NHS Foundation Trust is recruiting a Mental Health Practitioner to join the Crisis Resolution Home Team based at Hall Court, Telford. The role involves delivering specialist assessment and care planning, and working within a multi-disciplinary team to provide 24/7 crisis support.

As part of the Urgent Care Pathway, you will evaluate treatment plans, undertake home treatment where appropriate, triage referrals, and coordinate team activities.

Qualifications

• Registered Mental Health Nurse, Social Worker or Occupational Therapist.
• Deliver high-quality clinical service to patients and families.
• Work in a multi-disciplinary team within the Urgent Care Pathway.

Responsibilities

• Provide specialist assessment and care planning with service users and carers.
• Evaluate treatment plans and undertake home treatment.
• Triage and assess referrals in liaison with multi-disciplinary colleagues.
• Coordinate the work of the Team.
