Job summary

Devon Partnership Trust is recruiting a Community Support Worker to join the North Devon Crisis Resolution and Home Treatment Team. The role provides 24/7 community-based crisis resolution as an alternative to hospital admission and acts as a gatekeeper for inpatient beds.

You will work within an MDT, delivering home-based interventions and ensuring timely access to housing, benefits and local resources, while maintaining records and upholding trust values.

Qualifications

• Previous care experience in a caring role or mental health services.
• Extensive mental health experience with crisis interventions.
• Awareness of safeguarding principles.
• Experience working within a MDT.
• Ability to work autonomously.
• Experience monitoring physical health.
• Experience in psychiatric inpatient settings and crisis services.

Responsibilities

• Provide responsive, person-centred support to individuals in crisis in the community.
• Collaborate with clinicians, social workers and peer support workers.
• Deliver practical and emotional support to promote recovery and safety.
• Assist in crisis de-escalation and safety planning, face-to-face and by phone.
• Help service users access housing, benefits and community resources.
• Maintain accurate records in line with Trust policies.
• Participate in team meetings, supervision and training.
• Promote equality, diversity and inclusion in care.
• Work flexibly including evenings and weekends to meet pathway needs.
