Job summary

Essex Partnership NHS Foundation Trust is seeking a Social Worker to assess needs and develop, implement and evaluate care plans for Adults and Older Adults in the Crisis Resolution Home Treatment Team. The post reports to the Clinical Lead and works with multidisciplinary teams across wards and primary care.

It requires completing a preceptorship and delivering high-quality, person-centred care within an integrated service.

Responsibilities

• Holistic assessment of client problem/needs.
• Planning, implementation and evaluation of interventions.
• Clear, accurate and concise documentation of client care.
• Participation in initial assessment of clients for Crisis Resolution Home Treatment Service and liaison with referring agencies.
• Participation in Team and Clinical Meetings, initiating and discussing new methods/models of care and introduction of research or trials.
• Establishing effective risk management through clinical risk assessment and ongoing monitoring of mental state.
