Essex Partnership University NHS Foundation Trust is seeking a post holder for the Crisis Resolution Home Treatment Team to lead the assessment, planning and evaluation of care for Adults and Older Adults with mental health issues.

You will work within a multidisciplinary team, liaising with wards and primary care professionals, maintain care standards, and ensure an integrated service delivery that supports clients and colleagues alike. Preceptorship completion is required.

Qualifications

• Holistic assessment of client problems/needs required.
• Planning, implementation and evaluation of interventions.
• Clear, accurate documentation of client care.

Responsabilités

• Establish and maintain effective individual health and social care; holistic assessment, planning and evaluation.
• Participate in initial client assessment to determine suitability for Crisis Resolution Home Treatment Service and liaise with referring agencies.
• Join team and clinical meetings; discuss new models of care and introduce research or trials.
