Job summary

Codi Group is seeking a Complex Care Project Worker (Relief Bank) in Bedwas, Caerphilly. You will cover shifts for leave and sickness, supporting individuals on their recovery journey in a 24/7 service. This is a zero hour, relief post with potential 12-hour shifts up to 36 hours per week.

You’ll work as part of a dedicated team to deliver high-quality, person-centred care plans and liaise with relevant agencies. Training and development are encouraged as part of the role.

Qualifications

  • IT literate and capable of using basic software/tools.
  • Able to work effectively as part of a team.
  • Experience or knowledge of working with mental health or complex care needs.

Responsibilities

  • Support individuals to live independently within a care setting.
  • Prompt and administer medication as required.
  • Engage individuals in social activities and community links.
  • Complete and update support plans and risk assessments.
  • Undertake relevant training for the role.
  • Liaise with Social Services, Benefits Agencies, Housing department and others.
  • Organise group activities at Ty Anwen or in the community.
  • Provide emotional support to individuals.

About The Service

Tŷ Anwen is a residential complex care and specialist mental health service in Bedwas, Caerphilly, supporting up to 16 individuals living with serious and enduring mental illness, brain injury or other complex care needs.

Our team works to a recovery-focused model of care, offering clear therapeutic boundaries and outcome-driven support that builds independence and confidence.

About The Role

As a Complex Care Project Worker (Relief Bank), you will play a key role in covering shifts in the service for annual leave, sickness and other reasons. In the role you’ll be empowering and supporting individuals on their recovery journey. Working as part of a dedicated team, you’ll help to develop, implement and review high-quality, person-centred care and support plans that reflect each individual’s needs, preferences and aspirations.

This is a zero hour, relief post with the opportunity to pick up 12hr shifts, up to 36 hours per week. Our service runs 24/7 so you’ll be covering days, evenings, weekends and wakeful nights.

What You’ll Be Doing
  • Supporting individuals to live independently within a care setting.
  • Support, prompting and administering medication.
  • Engagement in social activities
  • Completing and updating support plans, risk assessments and risk management plans.
  • Undertake relevant training for the role.
  • Liaise with Social Services, Benefits Agencies, Housing department and other relevant agencies
  • Organising group activities with the individuals at Ty Anwen or in the community
  • Providing emotional support to the individuals
Skills And Experience Needed
  • I.T. literacy
  • GCSE Grade C or above in Maths & English (or equivalent)
  • Experience working as part of a team
  • Minimum of 2 years’ experience, or relevant knowledge of, working with individuals with mental health or complex care needs.
Pay And Benefits
  • £14.61 per hour
  • Sleep-in rate: £62.64 per 8 hour shift.
  • Social Care Wales registration is fully reimbursed
