Job summary

Pobl Group in Ty Anwen, Bedwas Caerphilly, is seeking a part-time Complex Care Project Worker to support residents with mental health and complex care needs. The role covers Thursdays, Saturdays and Sundays, totaling 24 hours per week, with a focus on activity engagement and recovery-focused support.

You will help implement person-centred care plans, assist with medication prompting, and coordinate group activities both on-site and in the community. Requires GCSEs and teamwork experience.

Qualifications

  • GCSE Maths & English at grade C or above (or equivalent).
  • Experience working as part of a team.
  • IT literacy is required.

Responsibilities

  • Support individuals to live independently within a care setting.
  • Support, prompting and administering medication.
  • Engage residents in social activities and plan group events.
  • Complete and update support plans, risk assessments and risk management plans.
  • Liaise with Social Services, Benefits Agencies, Housing department and other agencies.
  • Organise group activities with residents or in the community.
  • Provide emotional support to residents.

Job description

Part-time weekend work (Thurs, Sat, Sun) supporting activities at Ty Anwen , our specialist care service for residents with mental health, brain injury and other complex care needs. 24 hours per week. About the service Tŷ Anwen is a residential complex care and specialist mental health service in Bedwas, Caerphilly, supporting up to 16 individuals living with serious and enduring mental illness, brain injury or other complex care needs. Our team works to a recovery-focused model of care, offering clear therapeutic boundaries and outcome-driven support that builds independence and confidence. About the role This a specific role within our team at Ty Anwen. Working 24 hours per week, over three shifts, Thursday, Saturday and Sunday, you'll be a Complex Care Project Worker, but will be responsible specifically for organising activities and engagement with the residents too. Recent examples of events have included cinema trips, beach trips, a reggae themed night, so you can get creative and really work to provide opportunities that meet the interests of the residents. As a Complex Care Project Worker, you will play a key role in empowering and supporting individuals on their recovery journey. Working as part of a dedicated team, you'll help to develop, implement and review high-quality, person-centred care and support plans that reflect each individual's needs, preferences and aspirations. What you'll be doing:

  • Supporting individuals to live independently within a care setting.
  • Support, prompting and administering medication.
  • Engagement in social activities
  • Completing and updating support plans, risk assessments and risk management plans.
  • Undertake relevant training for the role.
  • Liaise with Social Services, Benefits Agencies, Housing department and other relevant agencies
  • Organising group activities with the individuals at Ty Anwen or in the community
  • Providing emotional support to the individuals Manual driving licence essential - for driving residents to events and .
  • I.T. literacy
  • GCSE Grade C or above in Maths & English (or equivalent)
  • Experience working as part of a team
  • Minimum of 2 years' experience, or relevant knowledge of, working with individuals with mental health or complex care needs.
Pay, Shifts and Benefits

Our service runs 24/7 so you'll be covering days, evenings, weekends and wakeful nights.

  • £14.61 per hour
  • Sleep-in rate: £62.64 per 8 hour shift.
  • Social Care Wales registration is fully reimbursed
