Job summary

We are seeking a Complex Care Project Worker (Relief) at Ty Anwen in Caerphilly. The role covers shifts for annual leave, sickness and other absences, delivering person-centred care and developing plans that reflect each individual’s needs and aspirations.

The position offers zero hours with potential 12-hour shifts, including days, evenings, weekends and wakeful nights. You will work as part of a dedicated team, providing support for independence, medication prompts, and participation in social

Qualifications

IT literacy
GCSE Maths & English (or equivalent)
Experience working as part of a team
Minimum of 2 years' experience working with individuals with mental health or complex care needs

Responsibilities

Supporting individuals to live independently within a care setting
Prompting and administering medication
Engagement in social activities
Completing and updating support plans, risk assessments and risk management plans
Undertake relevant training for the role
Liaise with Social Services, Benefits Agencies, Housing department and other relevant agencies
Organising group activities with the individuals at Ty Anwen or in the community
Providing emotional support to the individuals
