Qualifications

Responsibilities

• Be the first point of contact for the service, including taking referrals, doing assessments, covering staff absence and answering telephone queries.
• Provide continuity of care through effective partnership working, care coordination, key work responsibilities, and intervention delivery.
• Manage an active caseload and connect quickly with clients, ensuring they are treated with respect and dignity.
• Work collaboratively as part of a team to maintain a high-quality service.
