Job summary

Reed in Partnership is seeking a Neighbourhood Work & Health Coach to deliver personalised, person-centred support to individuals facing physical health, mental health or wellbeing challenges affecting employment. The role covers Chelmsford and surrounding areas, working with healthcare professionals and community partners to enable sustainable employment outcomes.

You will conduct biopsychosocial assessments, develop Work and Health Action Plans, and provide ongoing coaching to improve

Qualifications

Experience delivering personalised advice, guidance, coaching or support services to adults facing barriers to employment, health or wellbeing.
Experience managing a caseload and supporting individuals to achieve agreed outcomes.
Experience working in partnership with multiple organisations and stakeholders.
Strong understanding of how physical health, mental health and disability impact employment.

Responsibilities

Conduct comprehensive biopsychosocial assessments to understand health, wellbeing and employment circumstances.
Build trusted relationships with participants through a person-centred coaching approach.
Develop personalised Work and Health Action Plans including employment and wellbeing goals.
Identify barriers and coordinate interventions to enable sustainable employment outcomes.
Provide ongoing coaching and practical support to improve confidence and work readiness.
Coordinate referrals and support with MDTs, clinicians and community services.
