Job summary

Reed in Partnership is seeking a Neighbourhood Work & Health Coach to provide personalised, person-centred support to individuals facing health and wellbeing challenges impacting employment.

Role involves coaching, assessments, and case management to help participants stay in work, return after absence, or move into suitable roles, with strong cross-sector coordination and a focus on practical solutions.

Qualifications

• Experience delivering personalised advice, guidance, coaching or support services to adults facing barriers to employment, health or wellbeing.
• Experience managing a caseload and supporting individuals to achieve agreed outcomes.
• Experience working in partnership with multiple organisations and stakeholders.
• Experience working to performance standards and quality requirements.
• Strong understanding of how physical health and wellbeing affect employment.

Responsibilities

• Conduct biopsychosocial assessments to understand health, wellbeing and employment ties.
• Build trusting relationships using person-centred coaching.
• Use motivational interviewing to identify strengths, barriers and opportunities.
• Develop personalised Work and Health Action Plans with employment and wellbeing goals.
• Identify barriers affecting work capacity and progression.
• Provide ongoing coaching to improve confidence and work readiness.
• Support return-to-work planning and workplace adjustments.
• Coordinate interventions and referrals to address barriers.
• Provide employment coaching, including CV development and interview prep where appropriate.
• Collaborate with clinicians and community providers to coordinate integrated support.
