Job summary

Nottinghamshire Healthcare NHS Foundation Trust in Rainworth is seeking an Assistant Practitioner in Community Therapy and Pathway 1 (NHS AfC Band 4). The role supports the Mid Notts Discharge to Assess and Community Therapy Team, delivering goal-focused assessments and treatment plans to help adults regain independence.

You will work alongside physiotherapists, occupational therapists and support workers across a 7-day service, with shifts 8am–6pm, and supervision provided.

Qualifications

• NVQ level 3 in health and social care or equivalent qualification.
• Foundation degree or equivalent e.g. CPD units.

Responsibilities

• Create and deliver goal-focused treatment plans.
• Manage small non-complex caseload with support from a registered clinician.
• Maintain accurate records and assist with waiting lists management.
• Work with the team to prevent hospital admissions and support post-discharge.
