Nottinghamshire Healthcare NHS Foundation Trust is advertising for an Assistant Practitioner in Community Therapy and Pathway 1. The role involves delivering assessments and creating goal-focused treatment plans for adults 18+, with a 7-day service operating between 8am and 6pm.

The post supports discharge and community-based rehabilitation, under supervision, with DBS checks required and a pro-rated salary for part-time arrangements.

Formación

• NVQ level 3 in health and social care or equivalent qualification.
• Foundation degree or equivalent e.g. accredited CPD units.
• Experience of community health multidisciplinary working.
• Experience working with older adults in rehabilitation context.
• Full UK driving licence and vehicle for business use.

Responsabilidades

• Create and deliver goal-focused treatment plans.
• Manage a small non-complex caseload with support from a qualified clinician.
• Maintain accurate records and support waiting lists.
• Communicate effectively with patients and carers.
• Work within a MDT and professional boundaries.
