Job summary

Nottinghamshire Healthcare NHS Foundation Trust is seeking an Assistant Practitioner in Community Therapy and Pathway 1. The role focuses on delivering goal-focused rehabilitation with a team of Physiotherapists, Occupational Therapists and support staff.

The post works within a 7-day service between 8am and 6pm, with a rota for weekends and late shifts. DBS checks are required, and staff will be inducted with monthly clinical supervision.

Qualifications

• Excellent communication skills both digital and verbal.
• Experience with older adults.
• Ability to work in a team with registered staff.

Responsibilities

• Deliver patient assessments once competencies are completed.
• Create and progress goal-focused treatment plans.
• Manage a small caseload with support from a clinician.
• Maintain accurate records and support waiting lists.
