Job summary

The Lincolnshire Community Health Service NHS Trust is seeking an Assistant Practitioner to join Adult Community Therapy across The South East locality (Spalding, Boston, Bourne and surrounding areas).

You will assess patients in their homes or care homes, coordinate care with registered therapists, and support rehab and holistic needs to maximise independence. A foundation degree or equivalent is required.

Qualifications

• Foundation degree or equivalent is required.
• Experience in health or social care settings is desirable.
• Ability to travel across Lincolnshire and surrounding areas.

Responsibilities

• Independently assess, treat, review and discharge individuals using the Home First approach.
• Develop rehabilitation plans considering cognitive, psychological, social and medical aspects.
• Support independence in community settings with a person-centred approach.
• Work in a MDT and coordinate with carers/relatives where appropriate.
• Provide information on community services and assist access to these services.
• Maintain a caseload and monitor progress with supervision from a registered practitioner.
• Support discharge planning from acute and community settings.
• Contribute to project development to improve patient flow.
