Job summary

Lincolnshire Community Health Services NHS Trust is recruiting an Assistant Practitioner to join Adult Community Therapy across the South East locality (Spalding, Boston, Bourne and surrounding areas).

You will carry out holistic assessments in patients' homes or care homes, coordinate care with registered therapists and therapy assistants, and help meet rehabilitation goals to maximise independence. Travel around the county is required.

Qualifications

• Foundation degree or equivalent is required.
• Therapy experience is preferred but not essential.
• Ability to travel around Lincolnshire and Gainsborough areas.

Responsibilities

• Independently assess, treat, review and discharge individuals using Home First approach.
• Complete holistic assessments in patients' homes or care home settings to identify therapy needs.
• Co-ordinate care with registered therapists and MDT colleagues to meet rehabilitation goals.
• Maintain a caseload and provide high quality, patient-centred care while supporting discharge planning.
• Provide information on community services and support access to these services.
