Job summary

Qualifications

• Experience in community therapy settings is desirable.
• Ability to work as part of a multidisciplinary team.
• Good communication and organisational skills.

Responsibilities

• Contribute to delivery of therapy for patients with long term conditions and rehabilitation needs.
• Support integrated team works to facilitate early discharge and prevent unnecessary hospital admissions.
• Liaise with GPs, social services and other professionals to coordinate care and meet patient needs.
