Job summary

Cambridgeshire and Peterborough NHS Foundation Trust is seeking a Therapy Assistant Practitioner for a 3-month secondment to join our Intermediate Care Service in the Cambridge area. You will support recovery at discharge home, work with a multidisciplinary team, and develop skills in physical rehabilitation with education and career progression opportunities.

The post requires autonomous working under qualified therapists, with emphasis on patient goals and independence.

Qualifications

NVQ Level 3 or equivalent plus knowledge of patient conditions via training and experience.
Good general education up to GCSE or equivalent.
Evidence of relevant short course and in-service training.

Responsibilities

Assess, interpret, plan and implement therapy interventions for specific patient groups.
Use clinical judgement to evaluate patient progress and record measurements.
Provide advice to patients and carers.
Work autonomously under the direction of a registered practitioner within the Intermediate Care Team.
Keep the patient at the centre of care with carer participation in decision making.
Provide a high standard of care.
Work in community settings including lone working with access to Specialists when required.
Plan and prioritise own workload.
