Job overview

Wrightington, Wigan and Leigh Teaching Hospitals NHS Foundation Trust are currently looking to recruit enthusiastic and motivated qualified nurses to join our Out of Hours District Nursing team.

As the role involves visiting patient homes & clinics across the Trust every day, postholders must state in their application if they have access to a car with suitable business insurance. If you do not mention this in your application, you will not be considered for the post.

Monday to Sunday working pattern. This is for permanent part time posts (20 hours) covering evening shifts between 1800-2300.

Only those with active NMC registration can be considered for this post. Previous applicants need not apply.

Please note : This post is part-time and does not meet the minimum salary threshold required for sponsorship under the Skilled Worker visa route. Applicants who may require sponsorship are advised to check the current visa requirements on the GOV.UK website before applying. Eligibility will be on a case-by-case basis.

Main duties of the job

You will have a variety of clinical skills and be able to demonstrate continued professional development and qualified as a registered nurse. You will have highly developed communication and personal skills.

You will have the ability to work alone but still be a team player. If you feel you are adaptable, self motivated and able to respond to challenges then please apply for this position. This role requires an Enhanced DBS check + Adults' Barred Lists, which will be obtained during pre-employment checks.

You will need to be able to travel independently across the Trust.

  • To undertake the delivery of evidence based nursing care as delegated by the District Nurse Coordinator / Team Leader.
  • To undertake skilled clinical nursing procedures and provide clinical advice.
  • To provide nursing care and support for patients with complex and continuing health care needs
  • To assess, plan, implement and evaluate programmes of care according to the District Nurse assessment criteria.
  • To inform the District Nurse Coordinator / Team Leader of changes in the patient’s condition.

  • To support the District Nurse Coordinator / Team Leader in ensuring that the District Nurse Team works collaboratively and cooperatively with clinical colleagues.

Responsibility for Health & Safety

This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.

Responsibility for Management
  • To ensure that appropriate management information is available to the Team Coordinator / Team Leader and Clinical Manager.
  • To ensure that stock control is managed efficiently and effectively.
  • To ensure that the repair and maintenance of equipment in the absence of the District Nurse Coordinator / Team Leader.

  • To facilitate effective learning within community practice and nurture reflective practice.
  • To support the District Nurse Coordinator / Team Leader in creating an environment for continuing professional development to facilitate learning to ensure fitness to practice.
  • To identify the needs of the learner in terms of their individual learning requirements and set aims and objectives to ensure that their needs are met.
  • To attend mentor updates annually, if applicable.
  • To develop teaching packs, as required, to disseminate evidence based information to patients, carers, nurses and multi-agency professionals.
  • To deliver in-service training, as required, via various teaching methods.
  • To acknowledge the public health role and actively seek out opportunities for improvement and protection of the health and well-being of the wider community.
