Job summary

Nottinghamshire Healthcare NHS Foundation Trust is seeking an Assistant Practitioner in Mansfield to work with the Mid Notts Discharge to Assess and Community Therapy Team. The role involves goal-focused rehabilitation, collaboration with physiotherapists, occupational therapists and support workers, and delivery of patient-facing assessments after competency completion.

The team operates 8am–6pm across a 7-day service; duties include recording, safeguarding, and contributing to waiting lists.

Qualifications

• Essential: NVQ level 3 in health and social care or equivalent qualification
• Desirable: Foundation degree or equivalent e.g. accredited CPD units
• Good communication; Previous MDT working; Evidence of recent CPD

Responsibilities

• Prevent hospital admission, improve ADL function and support post hospital discharge.
• Speak to members of the public and adapt communication style.
• Understand falls prevention and rehabilitation; work with community,
• Provide care and support as required; maintain records and manage waiting lists
