The role involves delivering patient-centred reablement in community settings, under supervision of a registered professional, within a multidisciplinary team.

Qualifications

• NVQ Level 3 in Health and Social Care or equivalent.
• Relevant clinical skills.
• Competency Based Education Package - or prepared to undertake.
• Previous caring experience in a formal care setting.
• Experience in adult rehabilitation/caring role.

Responsibilities

• Work within the Discharge to Assess (Home First) within the Community Health Services Division.
• Recruit dynamic, flexible Rehabilitation Assistants to join this established service within South Nottinghamshire.
• Provide care to patients in a community setting to enable reablement and rehabilitation for daily living.
• Collaborate with a multidisciplinary team to deliver patient-centred care.
• Develop and monitor rehabilitation plans to support independence and reduce hospital admissions.
