Job summary

Norfolk and Suffolk NHS Foundation Trust is seeking a Band 4 Assistant Practitioner to join our Secure Services team at Northside House, Norwich. The post offers part-time, flexible working within a supportive MDT environment and requires a Care Certificate or Level 5 qualification and a Foundation Degree in Health & Social Care / Mental Health.

• Ability to manage time and meet deadlines.

• Conduct assessments and contribute to risk management.
• Develop care plans and coordinate delivery of care.
• Support Registered Nurses, Social Workers and Occupational Therapists in daily activity and recovery pathways.
• Participate in quality improvement or change management projects.
