Primary Integrated Community Services Ltd is recruiting for a Band 3‑level clinical support role within the ECNH service in Nottingham. The post involves working with GPs, nurses, therapists and care home staff to deliver proactive, personalised care for residents in nursing and residential settings.

You will undertake delegated observations, assist with ward rounds, contribute to care plans and support families and carers in understanding agreed steps.

Qualificações

• Level 3 qualification in Health and Social Care, Clinical Healthcare Support or equivalent knowledge and experience.
• Experience of providing direct care or clinical support in an NHS setting.
• Able to undertake delegated clinical observations and interventions following training and competency assessment.

Responsabilidades

• Prepare for planned care home ward rounds by collating resident information, observations and concerns.
• Contribute to personalised care and support plans by gathering information on mobility, nutrition, hydration, continence and wellbeing.
• Undertake delegated clinical observations (BP, pulse, temp, sats, weight, glucose) after training.
• Recognise changes from baseline and escalate to a clinician when needed.
• Support communication between PCN team and care home staff after reviews.
