Job summary

Worcestershire County Council is recruiting a Service Assessor to join the Reablement team in Worcestershire. The role involves completing initial community assessments, developing Promoting Independence Plans and actively case managing service users to support independent living.

You'll work a 37-hour week, with hybrid working where possible, and be supported by your line manager through regular reviews. A level 3 Diploma in Health and Social Care or substantial social care experience is

Responsibilities

• Undertake initial assessments within the community identifying needs and potential for regaining independence.
• Develop Promoting Independence Plans capturing outcomes and required interventions.
• Actively case manage service users within a locality as part of a multidisciplinary team.
• Ensure standards and performance measures are adhered to, including monitoring protocols and QA audits.
• Provide out of hours cover as part of a service delivery rota
