Job summary

Nottinghamshire County Council is seeking a Reablement Worker to support adults in their own homes as part of the START team. You will help people regain independence after illness or crisis, delivering compassionate, person-centred care in a hybrid working pattern.

Key duties include assisting with personal care, mobility, transfers, nutrition and medications; developing tailored support plans; collaborating with therapists; and maintaining accurate records in line with health and safety

Qualifications

• QCF Level 2 in Health and Social Care or willingness to work towards it.
• Full driving licence and access to a vehicle for home visits.
• Basic knowledge of safe working practices and dignity in care.

Responsibilities

• Support adults with personal care, mobility, transfers, nutrition, and medication.
• Encourage independence through tailored support plans.
• Collaborate with therapists and managers for safe, effective care.
• Monitor progress and provide feedback to the team.
• Communicate sensitively and professionally with people.
• Maintain accurate records and adhere to health and safety policies.
