Job summary
Care Dorset Limited is recruiting a Community Reablement Worker to provide in-home reablement support, helping individuals regain independence and improve daily living activities. You will work with a multidisciplinary team to deliver person-centred care and monitor progress, ensuring safety and quality aligned with CQC standards.
The role requires experience in community reablement, NVQ 2/3 in Health and Social Care (or willingness to work towards), strong communication and organisational
Qualifications
- NVQ Level 2/3 in Health and Social Care or equivalent.
- Experience delivering personal care and reablement in community/homecare.
- Ability to assess needs and develop person-centred care plans.
- Understanding of safeguarding and anti-discriminatory practice.
Responsibilities
- Provide reablement support at clients' homes to regain independence.
- Deliver personal care, mobility support and daily living activities.
- Assess needs, plan reviews, and update care plans as needed.
- Collaborate with multidisciplinary teams for holistic care.
- Monitor progress and document changes; escalate concerns promptly.
- Promote dignity, choice and wellbeing; maintain safeguarding standards.
