Overview

An exciting opportunity has become available to join our integrated multi-disciplinary team on a fixed term contract until 31/03/2026. The enhanced team west is looking for a registered mental health nurse who is enthusiastic in wanting to support people and their families/carers in a holistic way in the community, for those people who have complex needs.

The successful candidate will offer a person-centred approach to promote independence, well-being and support for people to live a fulfilling life in the community. The successful candidate will maintain a high standard of clinical work when carrying out nursing procedures.

Responsibilities
  • Have a caseload of people with individual care plans and positive risk management with support and input from specialist practitioners.
  • Be responsible for the safe custody of medication, including checking, administration and transportation of medication.
  • Ensure compliance with the Trust policy and procedure and clinical governance framework.
  • Knowledge and compliance with the Lone working policy.
  • Work in a multi-disciplinary team and maintain excellent communication with team colleagues and other services.
  • Be willing to work some weekends as the team provides a seven-day service.
  • Participate in the role of team duty, including delegation of daily tasks and supporting the team\'s flexible assertive community treatment approach (FACT).
  • Champion service user safety and dignity and drive continuous improvement.
