Job summary

Milestone Support Services, part of Natali Healthcare Solutions UK group, is recruiting Outreach Support Workers to provide person-centred help in homes and communities, enabling independence and meaningful daily activities in Gosport and Fareham.

You will join a supportive team, receive full training (PBS and complex needs), flexible school-friendly hours including evenings and weekends, and opportunities for progression. A driving licence and vehicle are required.

Qualifications

• Full UK driving licence and access to a reliable vehicle.
• Experience in care is beneficial but not essential.
• Ability to work independently and as part of a team.
• Good communication and interpersonal skills.

Responsibilities

• Providing support in individuals' own homes and within the community.
• Assisting with daily living activities.
• Promoting independence, choice, and dignity.
• Supporting individuals to attend appointments and social activities.
• Encouraging engagement in hobbies, interests, and community involvement.
• Maintaining accurate records and following care plans.
