Job summary

NHS Jobs in Chippenham area is recruiting a Band 5 Community Occupational Therapist to join our BSW Service. You will assess, plan and implement OT interventions in community and outpatient settings, promoting independence and wellbeing.

You will work with therapists, nurses and other professionals to coordinate care, maintain high standards of care, and undertake supervision and ongoing professional development in line with HCPC and Agenda for Change.

Qualifications

• Degree or Diploma in Occupational Therapy.
• HCPC registration is required.
• Full UK driving licence.
• Evidence of continuing professional development (CPD).

Responsibilities

• Assess, interpret, plan and implement occupational therapy interventions in community and outpatient settings.
• Develop and evaluate care plans; ensure documentation and review of care.
• Collaborate with a multidisciplinary team to coordinate care.
• Promote independence and wellbeing for patients and carers.
