Livewell Southwest is recruiting 2 x fixed term Band 5 Occupational Therapists to join our Community Therapy Teams in the South and East Neighbourhoods. The role involves comprehensive OT assessments, treatment planning, and collaboration with a multidisciplinary team to enable independence at home.

You will assess, prescribe equipment, and support housing adaptations, aiming to maximise people’s independence and participation in daily life within community settings.

Qualificações

• HCPC registration is required and ongoing professional development is expected.
• Experience with functional assessment, activity analysis and environmental adaptations is preferred.
• Ability to work in a MDT with community and hospital-based colleagues.

Responsabilidades

• Lead comprehensive OT assessments and treatment planning for adults.
• Prescribe adaptive equipment and coordinate minor/major home adaptations.
• Contribute to discharge planning and housing arrangements.
• Provide evidence for housing changes (e.g. Devon Home Choice) and coordinate community follow-up.
