Job summary

HCRG Care Group is recruiting a Band 6 Occupational Therapist for Melksham Community Hospital. The role focuses on comprehensive assessment, rehabilitation and discharge planning within a community-based Integrated Neighbourhood team.

You will work across home, residential and nursing settings, supporting independence and collaborative care planning, with flexible hours and weekend/on-call elements as needed.

Qualifications

  • Diploma/ Degree in Occupational Therapy is required.
  • Registration with HCPC and RCOT membership where applicable.
  • CPD portfolio and ongoing professional development are expected.

Responsibilities

  • Carry out comprehensive OT assessments, diagnosis, treatment and evaluation.
  • Focus on admission avoidance and timely discharge through MDT collaboration.
  • Manage a complex caseload and prioritise workload as service needs change.
  • Provide an evidence-based OT service including assessment, care planning, rehabilitation and discharge planning.
  • Must be a car driver with access to a vehicle for travel to domiciliary and community locations.
  • Supervise junior staff where applicable and participate in supervision.

Job description

Are you a motivated Occupational Therapist looking to make a real difference within your local community? HCRG Care Group is looking for a Band 6 Occupational Therapist to join our team in Melksham Community Hospital. The Integrated Neighbourhood team is a community-based service which supports and works with people, either in their own home or in Residential or Nursing Homes. The aim of the service is to maximise and maintain independence based on patients identified needs through rehabilitation. Following an initial assessment and, in consultation with the service user, a care plan and rehabilitation programme is drawn up. As our services transform, there may be times you will be expected to support both nursing and therapy in the delivery of care, within an appropriate scope or practice and following relevant on-the-job training. Flexible working across 24/7 to meet the service demands. Weekend working and out of hours working will be required.

Responsibilities and Requirements
  • To carry out comprehensive Occupational Therapy assessments, diagnosis, treatment and evaluation for patients with diverse presentations and a range of physical, psychological and social needs, using advanced clinical reasoning and person-centred goals.
  • Focus on admission avoidance, planned rehabilitation and facilitating timely hospital discharge through effective multidisciplinary and multi-agency working.
  • To hold responsibility for a complex caseload and manage capacity effectively, prioritising workload in response to changing patient and service needs.
  • To provide an evidence-based Occupational Therapy service of a high standard through comprehensive assessment, reassessment, care planning, rehabilitation, therapeutic intervention and discharge planning. Diploma/ Degree in Occupational Therapy
  • Membership of The Royal College of Occupational Therapists and/ or evidence of other professional liability insurance to practice
  • Registered with HCPC
  • Evidence of continuing professional development (CPD) recorded in a portfolio, as recommended by HCPC
  • To have gained effective experience in a range of practice areas
  • To demonstrate knowledge of principles and practices within Occupational Therapy to include knowledge of medical conditions prevalent in the elderly such as Parkinson's Disease, Stroke, MS, COPD, falls, orthopaedic conditions, general ageing and diabetes
  • Good working knowledge of typical work-related systems, procedures, equipment and technology
  • Knowledge and understanding of the legislation that applies to care and health work
  • Car driver with access to vehicle with business level insurance cover, business Insurance and be able to travel Independently
